History - Our pt was a 35 year old pt who was K/C/O CKD who presented with headache, seizures and hypertension.

MRI findings - There is flair hyper intense signals involving cortical-subcortical location of bilateral high frontal-parieto-occipital lobes

PRES is also known as acute hypertensive encephalopathy or reversible posterior leukoencephalopathy.

The term PRES can be a misnomer as the syndrome can involve or extend beyond the posterior cerebrum.

Supra Scapular Nerve Entrapment - Neuropathy By Ganglion Cyst

There is a large ganglion cyst seen in supraspinatus fossa -spinoglenoid notch causing likely compressing the suprascapular nerve.

We can also appreciate neurogenic intramuscular oedema in the supra and infraspinatus muscles supplied by this nerve.

Clinically it may be difficult to differentiate from arotator cuff tear. MRI Imaging helps.

Tuberous sclerosis is a neurocutaneous disorder (phakomatosis) characterized by the development of multiple benign tumors of the embryonic ectoderm.

Clinical triad of

  1. Seizures
  2. mental retardation
  3. adenoma sebacum

Radiological features :

  • Cortical-subependymal tubers, SEGA and white matter abnormalities
  • Renal angiomyolipoma
  • Cardiac rhabdomyomas
  • Lymphangioleiomyomatosis (LAM) in lung
  • Bony islands/cysts

Our patient of 1year old presented with seizures since birth

16 year old male, hospitalised for dengue before 1 week came with acute urinary retention.

MRI spine showed diffuse long Segment intra medullary hyper intensity in entire spinal cord suggestive of acute Demyelination
